The completed form may be faxed or posted to anyone you want, providing at all times, it is also forwarded to:
Your District 9455 Placement Coordinator.
Note that the address on the downloaded form is not the correct address to mail it to, your placement coordinator is the only address to which you should send your completed claim form.
